Job Title:  Medical Social Worker (MSW) – Home Health Full-Time or Part-Time

Location:   Burlingame , CA

We are currently seeking a  Medical Social Worker (MSW)to join our Burlingame , CA team. This role is key to supporting patients and their families as they navigate complex medical and social needs within the home health setting.

Responsibilities:

• Assess the psychosocial needs of patients and families to develop individualized care plans.

• Provide counseling and emotional support to patients and caregivers.

• Assist in connecting patients and families to community resources, financial assistance, and legal aid.

• Collaborate with nurses, therapists, and physicians to ensure comprehensive care planning.

• Document all services provided in compliance with agency policies and regulatory requirements.

Requirements:

• Master's degree in Social Work (MSW) –  Required

• 2+ years of MSW experience in a  Home Health or similar healthcare setting –  Required

• Strong interpersonal, organizational, and counseling skills

• Valid Driver's License and reliable transportation
